Getting more information is easy. Call our office at (541) 754-8417, and a staff member will be happy to answer your questions. Our office hours are 8:00 a.m. to 4:30 p.m., Monday through Friday. We can also provide information by mail or email. If you would like to tour the center, we recommend calling ahead so we can ensure staff is available to show you around and answer your questions.
My loved one is only 50 years old and recently finished therapy following a severe stroke. Is this just for seniors, or do you have younger adults in your program too?
Grace Center serves adults ages 18 to 100+. Many of our participants are under 65 and are living with stroke-related conditions, brain injuries, or other disabilities. Our Post-Rehabilitation Program is designed to help participants continue building on the progress made during therapy while supporting ongoing physical, cognitive, and social well-being.
My loved one has significant memory loss and requires a high level of care. Would they still be able to participate?
Most likely, yes. Each participant begins with an assessment so we can better understand their individual needs and determine the best level of support. Our Memory Care Program provides meaningful physical, cognitive, and social engagement for individuals living with moderate to severe memory loss, including Alzheimer’s disease and other dementias. Our experienced staff is trained to provide compassionate, person-centered care.
My loved one uses a wheelchair and needs assistance with transfers and toileting. Can Grace Center accommodate their needs?
Yes. Grace Center is equipped to support participants with mobility challenges. We have a mechanical sit-to-stand lift and trained staff who can assist with transfers, toileting, and repositioning throughout the day. We also utilize specialized mobility equipment, including a gait-harness ambulator, which may help some non-ambulatory participants safely practice walking and movement.
What are your hours of operation and where are you located?
Grace Center is open Monday through Friday from 8:00 a.m. to 4:30 p.m. and is closed on major holidays. Extended care before or after operating hours may be available by special arrangement for an additional fee.

The center is located near the Osborne Aquatic Center, one block south of Circle Boulevard between NW 9th Street and Highland Avenue.
Do you have attendance or scheduling requirements?
Participants are not required to maintain a fixed schedule, and families may choose the days and frequency that work best for them. However, to help us appropriately prepare staffing, meals, and activities, we ask that families notify us by 9:00 a.m. if they need to schedule or cancel attendance for that day.
Participants may attend as often as needed, and schedules can be adjusted as circumstances change. Our only general requirement is that participants attend at least once per month, excluding special circumstances.
